New patient forms

The packet is intentionally short. Every question on it changes something about how the visit runs or how a plan gets built.

• Patient information: legal name, date of birth, address, and the best phone number to reach you.
• Health history: prior operations, current conditions, allergies, and family history.
• Medication list: prescriptions, over the counter medicines, and supplements, with doses.
• Consent to treat and a notice of privacy practices acknowledgment.
• Release of records, if past testing or operative notes live with another practice.

Medication is the single most useful thing you can bring. Acid suppressing medicines, diabetes medicines, blood thinners, and anti inflammatory drugs all change surgical planning, and several of them change what testing is worth doing first.

The easiest way to get this right is to photograph every bottle, including supplements, and bring the photos to the visit. Doses and frequency matter as much as the names.

Forms can be completed before the visit or on arrival. Please do not send medical details through ordinary email. Use the contact form on this site only for scheduling questions, and bring completed paperwork with you or send it through the method the office confirms when your visit is booked.

Bring what you have. An incomplete history is normal at a first visit, and missing pieces are filled in with records requests. Nothing about the consultation depends on a perfect packet.
