Transoral Outlet Reduction (TORe)

An endoscopic suturing procedure for people who have had a gastric bypass. The outlet from the stomach pouch is made smaller from the inside, through the mouth, with no incisions.

Medical illustration of a small stomach pouch after gastric bypass joined to the small intestine, with an endoscope passed through the esophagus and a ring of sutures narrowing the opening between the pouch and the intestine.

What TORe is

After a Roux-en-Y gastric bypass, the small stomach pouch empties into the small intestine through an opening called the outlet, or the gastrojejunal junction. Over time that opening can stretch. When it does, food moves through faster, the feeling of fullness fades sooner, and weight regain can follow.

Transoral outlet reduction, or TORe, reduces the size of that outlet. Transoral means through the mouth. Working through the endoscope, sutures are placed around the outlet and drawn together so the opening is smaller. There are no incisions and no tissue is removed.

Dr. Good is one of very few surgeons in Colorado who performs both ESG and TORe.

Who it is for

People who have had a gastric bypass and are experiencing weight regain or a return of hunger, when the evaluation shows the outlet has stretched. People who want to address a prior bypass without another operation.

TORe is not right for everyone. The evaluation, including an endoscopy to look at the pouch and outlet, determines whether it fits.

What to expect

Before. A full evaluation, a review of your prior surgery records, and an endoscopy to look at the pouch and the outlet.

The day of. The procedure is performed under anesthesia. The anesthesia team will discuss expected comfort, awareness, recovery, and risks with you. Discharge timing and return to work depend on the procedure and your recovery. You will need someone to drive you home.

After. You move through a staged diet, starting with liquids and working back toward regular food. Follow-up is part of the treatment.

This page is educational information, not medical advice. What is right for you is decided in consultation, after a review of your history, your symptoms, and your testing.

Common questions

Is TORe another surgery?
No. It is performed entirely through the mouth with the endoscope. Nothing cuts through the skin.
Is TORe the same as ESG?
Both are endoscopic suturing procedures. ESG reshapes a stomach that has not had bariatric surgery. TORe reduces the outlet of a stomach pouch after a gastric bypass.
Do I still need to change how I eat?
Yes. TORe works with your effort, not instead of it.

Next step

Start with the evaluation of your pouch and outlet.
